Set up lead forms for location contact

What this article helps you do

Enable a lead form on your store locator, choose the fields customers see, and send submissions to the right email recipients. Customers can use the form in the store-locator flow when no results are found.

Before you start

  • You have at least one store with an address in Mapstore
  • The Mapstore map block is added to your theme (typically on a page template)

Step-by-step instructions

1. Open Lead Generation

In Mapstore, go to Lead Generation in the main navigation.

2. Set the form status to active

The form has two states: draft (hidden) and active (visible). Use draft while editing, then switch to active when you are ready for customers to see it.

3. Edit the form heading and text

Customize the heading (e.g. “Get in touch”) and the short text below it. The form text should explain what the form is for (e.g. “Have a question about this location? Send us a message.”).

4. Choose your fields

The default form includes name, email, message, and a dropdown (e.g. “How did you hear about us?”). In the field editor you can:

  • Add fields you need, such as phone, city, or subject
  • Remove fields you do not need
  • Set clear labels and placeholder text
  • Mark important fields as required

Keep required fields minimal so the form stays easy to complete.

5. Configure email notifications (optional)

Go to Settings to add notification emails. Each lead submission is sent to your shop email plus any extra addresses you add (up to 10).

Use valid, monitored email addresses. If your store uses Shopify Flow for lead routing, keep the Flow enabled and test it after changing the form.

6. Test the form

Submit a test lead from the storefront. Confirm the email includes the customer’s name, message, and any extra fields you collect.

How to verify it worked

  1. Save the form and set status to active.
  2. Open your store locator page on the storefront.
  3. Run a search that returns no results and confirm the lead form appears in the list panel.
  4. Submit a test message and confirm you receive it by email.

Common issues and fixes

IssueFix
Form does not appearConfirm the form status is active, not draft. Save changes and refresh the storefront.
Form appears but no email receivedAdd your shop email in Settings and ensure notification emails are configured. Check spam/junk folders.
Team members do not receive leadsAdd them in Mapstore notification emails, up to 10 addresses, then save and submit another test.
Required field is not enforcedMake sure the field is marked as required in the Lead Generation settings.
Submissions are missing dataCheck that the field is still included in the form and saved.
Shopify Flow does not runConfirm the Flow is enabled and uses the Mapstore lead submission trigger.
Map block not on store locator pageAdd the Mapstore map block to the page template in the theme editor.
